Our Settling-In Service provides short-term support to people who have recently been discharged from hospital and have been identified by the hospital team as having no further clinical needs, but who may benefit from a little extra support to settle safely back at home.
We provide support for up to two weeks, with up to three contacts during this period. Our aim is to make sure the person is managing well, feels safe and supported, and has everything they need to maintain their independence at home.
Support can include:
- π Shopping support where someone is struggling to get essential groceries.
- π Prescription collection to help ensure medication is available following discharge.
- π€ Carer support where a family member or unpaid carer may need additional help.
- π Welfare and settling-in calls to check how someone is managing at home.
- π Signposting and referrals to other appropriate community services where additional support is identified.
The service takes a person-centred approach, responding to what matters most to each individual and their family or carer. Sometimes a simple phone call, a little practical help or connecting someone with the right community service can make a significant difference following hospital discharge.
By providing this short period of support, we aim to help people settle back into their homes with confidence, promote independence and wellbeing, and reduce the risk of avoidable difficulties or readmission to hospital.
Our Settling-In Service is here to provide that extra reassurance when someone needs a little helping hand after coming home from hospital.
