Where people matter most

All our services are FREE – Supporting our community to live well for longer.

2024 marked the 30th Anniversary of HCHfH serving the community of Hull. Its a great achievement to have marked30 years of making a difference to people’s lives.

Home from Hospital (HCHfH) was established in 1994, originally to help those recovering at home after a hospital stay who have little or no support. Today HCHfH also offers services for families affected by a life limiting illness, carers, refugees and the lonely or isolated. The charity provides its services through the kindness of volunteers who give their time and care for free, but its work is recognised and supported by the NHS, LA and other care organisations with which we work very closely.
